The upper right central incisor is isolated and thin braided retraction cord impregnated with a non-staining agent is in place. Note the Lanolelle on the approximating contact surfaces to prevent adhesive locking of the tooth. The shining surface indicates that a dentin bonding agent has been applied.

A 360 degree extrusion of the resin cement is seen around the margins.

After removal of the excess cement, the retraction cord is removed with flecks of resin cement attached.

  1. Use local anesthesia and remove the provisionals. Place a thin retraction thread to restrict crevicular fluid flow (a non-staining material like Hemodent is preferred).
  2. Sandblast the prepared tooth structure to remove remnants of temporary cement.
  3. Try-in the crown:
    • check seating and marginal fit. On occasion, Fit-Checker may be needed.
    • relieve the interfering area on the tooth.
    • cleanse the silicone residue from the crown with acetone and sandblast the tooth again (silicone residue on tooth and crown prevents adhesion).
    • check the contacts with floss. If tight, gently relieve and polish to a high gloss with Brasseler porcelain polishers.
  4. Check occlusion with red and green ribbons, in centric and eccentric jaw movements, including protrusion. Make appropriate adjustments and polish.
  5. Check phonetics and adjust as appropriate.
  6. Rely X ARC from 3M is the cement of choice. It provides durable long-term chemical bond with the densely sintered aluminum oxide core material. Little shade manipulation is possible with Procera.
    • Sandblast internal of crown. DO NOT APPLY SILANE.
    • Mix and place ED primer, or appropriate dentin bonding agent, on tooth. Some require acid etching. NOTE: surface must be shiny.
    • Place Lanolelle on the approximating contacts to prevent adhesion.
    • Place Rely X inside the crown and carry the crown to the mouth on small wax-tipped carriers. There must be an extrusion of cement around the entire periphery.
    • Seat firmly and maintain constant seating contact. Apply Oxygard to exclude air and initiate the chemical set.
    • Wait three minutes and use #12 blades on Bard Parker instrument to remove excess. Then trim with KPC6 Curved Carbide Tipped Finishing Knife from Bisco. Remove thread.
    • Check and adjust occlusion.
    • Polish with Brasseler kit and diamond paste.
    • Remove thread.
  7. Photograph and enjoy!
